Shanti Chapman, TCM

As a targeted case manager at Counseling Associates of Lexington, I am dedicated to helping those in need. I have a master's degree in piano performance from Marshall University. Working directly with the homeless in Richmond, KY, as a peer support specialist and then being promoted to a behavioral health associate, I gained a solid foundation in understanding human behavior and the challenges faced by those experiencing homelessness and/or struggling with substance use. I am actively pursuing my master’s in social work from Eastern Kentucky University. In my role at Madison Home, my primary goal is to connect clients with important local resources that meet their basic needs, such as housing, income, and Medicaid or SNAP benefits. Based on my experience working with the homeless, I approach case management with a strengths-based therapeutic approach. By focusing on the strengths and resilience of individuals, my goal is to help them overcome obstacles and achieve their goals. I want to make a positive difference in the lives of my clients and am committed to constantly improving my knowledge and skills to provide the best possible support.

Brianna Reeves-Putthoff

Madison Home welcomes Certified Peer Support Services at 5th by Brianna Reeves-Putthoff. A Peer Support Specialist is someone, who has personal lived experience struggling with Substance Abuse and Mental Health Disorders in recovery. Now her journey is to help others like her to get them the help they deserve! She would be at 5th on Mondays from 11:30 am to 1:00 pm. Her assistance includes free Narcan, and links to resources and services including detox, medication-assisted treatment, residential treatment/rehabilitation, outpatient programs, and other resources in Madison County.

Madison County Harm Reduction Services

Harm Reduction staff and the mobile van will 5th Street on Wednesdays 11 am – 1 pm.

The team provides Free Norcan and the recovery coach from Voices of Hope may be available during this time.
